employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to:
A crushing hazard.  Employees face the potential of
being thrown from a fork truck due to the lack of seat
belt usage observed in the warehouse area.
Among other methods, one feasible and acceptable method
to control the hazard is to develop and implement some
form of disciplinary actions to ensure that employees
wear seat belts while operating fork trucks.
Abatement certification is required for this violation. The
abatement certification sheet is enclosed with the
citations.ion
