PLANT-WIDE:  SEAT BELTS WERE NOT PROVIDED FOR EMPLOYEES WHO
OPERATED FORKTRUCKS AROUND CORNERS AND ON RAMPS.
ONE FEASIBLE AND USEFUL METHOD TO CORRECT THE HAZARD, AMONG
OTHERS, IS TO PROVIDE AND ENFORCE THE USE OF SEAT BELTS ON SIT DOWN
POWERED INDUSTRIAL TRUCKS.

were exposed to the unexpected release of a suspended load due to crane
component failure:
HEAT TREAT:  THE P&H 3 TON UNDERHUNG MONORAIL CRANE HAD A DAMAGED
WIRE ROPE AND A WORN SHEAVE.
ONE FEASIBLE AND USEFUL METHOD TO CORRECT THE HAZARD, AMONG
OTHERS, IS TO:
1.HAVE A COMPETENT PERSON/COMPANY THOROUGHLY INSPECT THE
CRANE AND PERFORM ALL NEEDED MAINTENANCE AND REPAIRS.
2.DEVELOP AND IMPLEMENT A PROGRAM OF REGULAR DOCUMENT CRANE
INSPECTIONS INCLUDING MONTHLY WIRE ROPE INSPECTIONS AND AN
ANNUAL INSPECTION OF THE ENTIRE CRANE WITH CORRECTIVE ACTION
TAKEN AS NEEDED.
