Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On and prior to 3/7/97, employees were exposed to serious injuries while
conducting steel coil
banding operations on the #248 Automatic Feed Co. Uncoiler/Straightener
equipment.
Among other methods, the following are feasible and acceptable methods to
minimize the
probability of injury during banding operations:
(1)  Develop safe banding procedures which list the sequence of
operations. The procedures must
also include the control panel settings needed to keep the hold-down arm
down during the
banding operation, a warning to employees to stay well clear of the
uncoiler end, and to place
the Coil Car under the coil prior to banding operations.
(2)  Train all affected employees on the requirements set forth in the
procedures and related
safety precautions.
(3)  Enforce employee use of the procedures.
The following are ANSI B11.18-1992 recommendations for proper banding:
-  Banding material should be a minimum of 1-1/4" wide x .031" thick, heat
treated.
-  All banding seals should be double crimped.
-  Corner protectors should be used on all bands placed through the eye of
the coil. They should
be sufficient enough to protect against rupture of the band due to the
sharp edges of the material
or too acute a bend.
- A minimum of 3 bands (equally spaced) should be placed through the eye
of
the coil.
Bands should be placed on the circumference to the coil as follows:
Place a minimum of one band (centered) on coils up to 12" in width; for
coils over 12" in
width, place a minimum of two bands (equally spaced).
