29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Existing Lockout procedure was not utilized for protection of employees from machine hazards which were not eliminated either by effective guarding or de- energizing of the equipment. The employee clearing the discharge chute of the Urschel RA-A Dicing machine #2, Serial #1187 on January 27, 1997, was not protected by disconnecting from the power source or locking out, exposing that employee to contact with the rotating crosscut knives.t
