Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing, or likely to cause, death or serious physical harm to
employees in that employees
were exposed to a fall hazard:
Employee was exposed to a fall hazard from the swivel, approximately
eighteen
(18) feet above the rig floor, due to the safety belt lanyard not being
secured to
an anchor or structural member capable of supporting a minimum dead weight
of
5400 pounds as required by API RP 54-92 Section 4, Sub-Section 4.5,
Paragraph
4.5.1, page number 8, in that the employee had secured his safety belt
lanyard
to the gooseneck of the swivel, and in the process of changing out the
swivel
packing gland unit, all eight (8) bolts securing the gooseneck and swivel
packing
gland unit were removed, thus allowing the gooseneck and kelly hose to
fall,
taking the employee with it.h
