Employee was exposed to the hazard of fire and explosion due to the
failure to
erect a Geronimo Emergency Escape Line with a properly installed Geronimo
Escape Device at Rig Number 10:
This occurred on or about May 20, 1997 on San Patricio Corporation, Rig
Number 10. Among other methods, one feasible and acceptable abatement
method
to correct this hazard would be to install a properly rigged and secured
emergency descent or other satisfactory auxiliary means of egress,such as
specified in Section 5, Sub-section 5.7, paragraph 5.7.1 of American
Petroleum
Institute (API) RP 54-92, page 12. Essential elements of this procedure
would
include:
a.Staking out a securely anchored one-half inch wire rope escape line
attached to the derrick or mast so as to provide a ready and convenient
means from the Derrickman's working platform.
b.The escape line route is to be kept clear of obstructions.
c.A safety device is to be provided to permit fast emergency exit from the
Derrickman's working platform to a safe place.
d.The tension on the escape line is to be periodically checked and adjusted
to assure a safe landing and to allow the Derrickman to stop 20 to 25 feet
from the anchored point.
e.The ground anchor point of the escape line is to be located a minimum
lateral distance from the derrick or mast equal to the height above ground
level to the point where the escape line is fastened.

Employees were exposed to the hazard of being struck by a high pressure
flowline due to the failure of securing the flowline.
This occurred on or about May 21, 1997 on San Patricio Corporation Rig #
10.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard would be to secure the flowline to the ground. Essential
elements of
this procedure would include:
a.Secure or anchor the flow line at the end of the flow line and at
intermediate intervals along the line each 15 to 20 feet.
b.Where chis-san swivel connections are used to make a turn in a flow line,
the flow line shall be anchored to the ground by means of chains, steel
stakes or other adequate restraint on both ends of the chic-san swivel
connection.
c.That employees shall not be permitted to step across and/or on a
pressurized flow line. This would allow for protection of employees who
are working in close proximately to the pressurized flow line.
