seat belt:
(a)On or about May 20, 1997 and times prior thereto, a operator of a Toyota
forklift did not wear the seat belt when operating his forklift in the
storage
yard.  The operator was exposed to injury from being thrown from the seat
if
a wheel dropped in a hole or being caught between the cage and the pavement
should the forklift tip over as the yard had dips and holes in the paved
surface.

(a) In the department laying out sheetrock for ceilings, on May 20, 1997
and
times prior thereto, two employees hoisted and left suspended, an assembled
ceiling and started the sheetrock layout for the next ceiling under the
suspended load .  The employees were subjected to crushing injuries from
the
ceiling suspended by the overhead hoist because the suspended ceiling was
not
resting on blocks, jacks or other appropriate supports.  The operation of
the
overhead hoist was not in accordance with American National Standards
Institute (ANSI) B30.16-1987, Safety Code for Overhead Hoists, section 16-
2.3.2.3.f
