Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falls to the ground below:
On the north side of the facility, an employee was exposed to the hazard
of falling to the ground
when he was lifted up in a metal tub to access an outdoor light
approximately 15 feet high and
the employee did not wear protective tether attachments and the platform
did not have
appropriate restraining characteristics.
One method of abatement to eliminate the hazard is to ensure all platforms
used to lift employees
have restraining means such as rails, chains, or cables positioned to a
height not less than 36
inches measured from upper surface to floor of the platform; withstand a
concentrated,
horizontal 200 pound load applied at the point of least resistance; and
ensure
the employee lifted
uses a safety harness and lanyard as required by ANSI B56.1-1975, Safety
Standard for Powered
Industrial Trucks.
