On or about 9-23-98, employees were involved in post tensioning one-half
inch cables with a
"Power Team" post tensioning machine model C, serial number 391912,
located at the east side
of the parking deck at the second level near column E-8.  Employees were
exposed to the hazard
of being hit by the cable or post tensioning machine components, as they
were not removed from
the vicinity prior to reducing pressure on the cable.
"Among other methods one feasible and acceptable abatement method would be
to remove
personnel from the immediate vicinity of the cable or post tensioning
machine components prior
to releasing tension on the cable in accordance with the Post-Tensioning
Institute
