were exposed to burns from molten metal coming in contact with puddles of
water:
East side lab. Molten metal was transported by forklifts adjacent to an
area where the air
conditioning unit had created puddles of standing water.
Among feasible abatement methods is to fix the leak and dry the area with
absorbent material.

were exposed to falling loads from failure of a custom made lifting hook:
a.Mark V Melt. Employees used a custom made lifting hook to transport 1800
lbs. starter
blocks. No capacity was marked, no proof testing was performed.
Among feasible abatement methods is to comply with the American National
Standard for Below
the hook lifting devices, ANSI B30.20 and:
1.Provide a lifting device that is designed with a safety factor of three;
2.Marked with the rated capacity.
3.Proof tested to 125of the rated capacity.

unexpected nitrogen purge:
a. Mark V Pressure Pour line 6. No warning for nitrogen purge to protect
pour operator
rodding the spout or worker who may be unde the area for repairs.
b.Mark V Pressure Pour line 5. No warning for nitrogen purge to protect
pour operator
rodding the spout or worker who may be unde the area for repairs.
Among feasible abatement methods is to install a pre-purge alarm that
would warn warn
employees of imending nitrogen purge so that they could leave the area
toward a safe distance.

furnace must be shut down due to unsafe reduction in the furnace lining.
Among feasible anatement methods is to:
1. Determine the maximum reduction allowed for a furnace lining.
2. Monitor and document reduction in the furnace lining weekly.
3.Shut down furnaces when lining reduction at or below minimum thickness.
