Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The hazard of equipment failure in that the employer did not institute a policy and procedure for conducting periodic and regular inspections of horizontal injection molding machines, and did not provide adequate training to ensure that the operators and supervisory personnel are trained and instructed in the safe method of operation of safety devices and procedures. Feasible means of abatement include: (a) Establish a complete program of periodic and regular inspections of horizontal injection molding machines in accordance with ANSI/SPI B151.1 1997, section 4.3. (b) Ensure operators and supervisors are trained and instructed in the safe method of operation of the safety devices and operations as described in ANSI/SPI B151.1 1997, section 7.1.ion
