Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)Loading dock ( trailer and stock room ); damaged and improperly
assembled
straight gravity skatewheel conveyor was used to unload a semi trailer
truck. The
conveyor had collapsed on several occasions due to improper bracing or
anchoring and improper attachment of the legs to the conveyor sections.
Members
of the conveyors were bent, broken or missing.
Among other methods, feasible and acceptable abatement methods to correct
this
hazard, include but are not limited to:
1.Inspect each conveyor component and replace or repair as needed.
Safety Standard of Conveyors and Related Equipment ANSI B20.1-
2000 5.2(b) and 5.12(h)
2.Insure proper assembly procedure per manufacturer's direction. This
may include but not limited to:  ANSI B20.1-2000 5.12(g)
*the lagging the conveyor to the floor where possible,
installation of knee braces,
*two legs per section,
*positioning the leg between each section and bolting securely,
*Provide adequate size and numbers of bolts, washers and tools
for securing legs to the frame and
*place the hook section on the upstream side of the conveyor
section.
3.Train employees in proper assembly procedure. ANSI B20.1-2000
5.2(b) and 5.12(a).
4.Establish a maintenance and inspection program to ensure the
integrity of the components. ANSI B20.1-2000 5.2(b)
