SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  Employer knew or should have known that surveyors entering sewer
manholes
that had not been evaluated as being permit-required confined spaces were
exposed to the hazards associated with unknown atmospheres such as hydrogen
sulfide, methane gas or oxygen deficiency.  A feasible and useful method to
correct this hazard, among other methods, is to evaluate the worksites for
confined spaces.
B.  Employer knew or should have known that surveyors entering sewer
manholes
that had not been evaluated as being permit-required confined spaces were
exposed to the hazards associated with unknown atmospheres such as hydrogen
sulfide, methane gas or oxygen deficiency.  A feasible and useful method to
correct this hazard, among other methods, is to develop written procedures
for
non-entry work, disciplinary actions when policy is not followed, and
refresher training.
C.  Employer know or should have known that surveyors entering sewer
manholes
that had not been evaluated as being permit-required confined spaces were
exposed to the hazards associated with unknown atmospheres such as hydrogen
sulfide, methane gas or oxygen deficiency.  A feasible and useful method to
correct this hazard, among other methods, is to include in the company's
own
confined space program, a policy to address hazards and doing work in sewer
manholes.
