SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code
of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  On December 12, 2002 employer knew or should have known that two
supervisors and two workers were exposed to the hazards of being struck by
falling equipment while moving an unstable load with a Taylor forklift,
Equipment Number SDG-21406, onto a moving Talbert flat bed trailer in the
parking lot of Steel Parts, Inc.  A feasible and useful method to correct
this
hazard, among other methods, is to only move stable and safely arranged
loads
with the forklift and to keep all employees out of harm's way.

properly secured or chained to the lift.  A feasible and useful method is
to
only move loads that have been chained or otherwise secured to the
forklift.

Laws, State of South Carolina, 1976, as amended, as follows:
A.  On December 12, 2002 employer knew or should have known that two
supervisors and two workers were exposed to the hazards of being struck by
machinery or truck while loading a trailer while it was being moved, the
brakes were not set and the wheels were not blocked.  A feasible and useful
method to correct this hazard, among other methods, is to always have the
brakes set, the wheel blocked and to never allow a driver to move the
vehicle
while it is being loaded with workers standing in harm's way.
