SCRR 71-112A: Failed to furnish a place of employment which is free of
recognized hazards which may cause death or serious physical harm to his
employees and comply with this regulation and other occupational safety and
health rules and regulations promulgated under Chapter 15 of Title 41,
Code of
Laws, State of South Carolina, 1976, as amended, as follows:
A.  Employer knew or should have known that employees who were installing
grooved end pressure piping at Clemson University's Ravenel Center were
exposed to the hazard of being struck by the pressurized piping and
couplings
that they were working on.  A feasible and useful method of abatement,
among
others, is to develop and implement written procedures for installing and
leak-testing grooved end pressure piping which shall state that before any
employee attempts to adjust, remove, or install any grooved end piping or
couplings, the entire piping system shall be depressurized to "0" psi and
all
appropriate valves shall be locked out to prevent unintended
repressurization
of the piping system, which adjustments are made to any grooved end piping
or
couplings.
