recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls into elevator shaft:
a)Mold Storage Area - Two story freight elevator shaft was not adequately
guarded on one side to prevent employees from falling approximately 9
feet when elevator is on lower level. Side in question had only a single
chain across opening.
ABATEMENT NOTE:
Among other methods, feasible and acceptable abatement methods to correct
this hazard
are to guard the open sided elevator shaft according to ANSI A17.1 - 1971,
Elevators,
Dumbwaiters, Escalators, and Moving Walks.
