31-372-107 Section 31-370(a) of the Connecticut Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazardous working conditions prohibited by ANSI/ASME B30.9- 1984 section 9-1.5.4, "Makeshift fasteners, hoofs,or links formed from bolts, rods, etc... or other such attachments shall not be used." Location: (A)Town of East Hartford, Department of Public Works, Highway Department(site address, 1450 Main Street) (1)Employer whose employees used chain slings for lifting a 5500 lbs, pre- cast concrete dry well, did not ensure that an old street sign post was not used as a makeshift attachment while lifting the load. Feasible methods of abatement for this hazard: (1)Develop and implement a sling safety program and training procedures for employees who work with chain slings and attachments. (2)Employees shall be instructed on how to recognize and avoid unsafe working conditions and hazards when using chain slings and attachments. (3)Avoid the use of makeshift attachments when using chain slings.
