recognized hazards that
were causing or likely to cause death or serious physical harm to

were exposed to fractures and crushing injuries due to the collapse of an
unsafe structure:
Kwik Car Wash - On or about February 12, 1998 employees were working
underneath
the overhead canopy that had weakened and collapsed due to the
accumulation of frozen
and unfrozen water.  The collapse resulted in the death of one employee.
ABATEMENT NOTE:  One feasible method to abate this may be to remove
accumulations of
frozen and unfrozen water when the design limitations of the roof are
exceeded.ns
