recognized hazards that were causing or likely to cause death or serious

harm to employees in that they were exposed to being struck by and crushed
between the superstructure of an excavator and its tracks.

Employees were walking and working next to the side and rear of the
Komatsu Crawler Excavator while the machine was in operation.
Among other methods, one feasible and acceptable abatement method
to correct the hazard is to install a physical barrier or warning line
around the swing radius of the excavator to prevent employee access
into the danger area. This is an acceptable industry practice when the
operator is not constantly repositioning the machine.
