Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to work conditions which increased the possibility of physical injury: jobsite: employees were not adequately trained in the safe operation of the Snorkel Economy Wildcat Series 1200, dual wheel hydraulic drive, self-propelled elevating work platform, model #BC-2133-2W, serial #EK77622. Among other methods, one feasible and acceptable abatement method to correct this hazard includes, but is not limited to, ensuring that all employees read and understand the manufacturer's instructions and safety rules and are fully trained in the operation of self- propelled elevating work platforms in accordance with ANSI standard A92.6-1979, which includes, but is not limited to, a thorough inspection of the equipment and the work area prior to use to ensure that: 1/ the work platform will travel and be used on a firm and level surface 2/ the load and its distribution shall never exceed the manufacturer's rated capacity 3/ the work area and path of travel is free of debris and obstructions 4/ employees inspect functional controls for proper operation and that safety devices are not altered or disabled 5/ employees read and understand all decals, warnings, and instructions in the safe operation of the work platform 6/ employees inspect for defects such as hydraulic leaks, loose wire connections, and tire damage on a daily basis

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to serious physical injuries from an elevated platform that was extended beyond the maximum travel height: jobsite: on April 28, 1997, two employees were exposed to fall hazards of 20 feet 2 inches from a Snorkel Economy Wildcat Series 1200, dual wheel hydraulic drive, self-propelled elevating work platform, model #BC-2133-2W, serial #EK77622, due to a travel height limit switch that was rendered inoperable. Among other methods, one feasible and acceptable abatement method to correct this hazard includes, but is not limited to, ensuring that all safety devices are functional for proper operation and are not in anyway altered or disabled in accordance with ANSI standard A92.6-1979 and the manufacturer's operating/maintenance manual.
