Employees installing and rigging a two-point suspension scaffold for
building and
window washing were not tied off while walking and working on the roof,
creating a fall hazard of up to fifty feet from the roof.
Among other methods, one feasible and acceptable method of abatement to
correct this hazard includes, but is not limited to, ensuring employees
are trained
in recognizing and avoiding fall hazards associated with roofs and other
elevations, and instructed in the procedures to be followed to minimize
these
hazards and ensure that safe working practices are observed.
