Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall hazards:
(a) Facility, Building #2 and #3:  A welding slag bucket, attached to the
Demag
overhead crane, was used to lift employees in order to inspect jib hoists.
As a
result of this practice, the following deficiencies occurred:
1.  The company used a crane to move personnel when other
feasible methods could have been used.
2.  The welding slag basket and suspension system were not
designed by a qualified engineer or a qualified person competent
in structural design.
3.  There was no plate on the welding slag basket nor permanent
marking which indicates the weight of the basket and its rated load
capacity or maximum intended load.
4.  There was no closing device on the hook of the 60 ton
overhead Demag crane.
5.  There was no trial lift conducted.
6.  A body belt/harness system was not used by the employee in
the welding slag basket.
7.  The overhead 60 ton Demag crane was traveling with the
employee in the welding slag basket.
8.  There was no pre-lift meeting.
