Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees who were exposed
to steam, fire and explosion and hazards:
(a) Boiler room;  Workers who operate, inspect, repair, service and
interchange
equipment and parts on the three "Titusville" automatically fired natural
gas
boilers were not properly trained nor were they certified to perform these
functions.
Among other methods one feasible and acceptable method of abatement is to
ensure that workers receive training and education through examination and
certification in accordance with the American Petroleum Institute, API
510, and
the applicable portions of Sections V, VIII and IX of the latest edition
of the
American Society of Mechanical Engineers, ASME Code and ANSI NB-23.

to fire, explosion and steam hazards:
(a) Boiler room;  There was no documented and implemented quality control
system for the three "Titusville" automatically fired natural gas boilers
that were
converted from oil to natural gas.  The control panels were altered with
"Wagner" lockout boxes.  Equipment and parts such as, the main fuel gas
regulators, welding on the steam piping system, repaired combustion
chambers,
adjusted damper arm linkage for air/fuel ratios were not documented on a
quality
control and inspection program.
Among other methods, one feasible and acceptable means of abatement is to
implement a documented quality control program for materials, parts,
repairs and
inspections as outlined in accordance with API 510.

were exposed to fire, explosion and steam hazards:
(a) Boiler room;  Preventative maintenance inspections were not conducted
on a
frequent and periodic basis nor were permanent records maintained on the
three
"Titusville" automatically fired natural gas boilers that were converted
from oil
fired.  Interchanged equipment and parts, repaired combustion chambers,
adjusted
air/fuel gas ratios for the main and pilot gas valves, altered control
panels, a
clogged drip leg and a partially opened hydramotor valve were on the
boilers.
Among other methods, one feasible and acceptable means of abatement is to
initiate a progressive and permanent record system for modifications,
alterations,
materials, parts, and repairs as outlined in accordance with API 510, ASME
CSD-1-1995 and the National Boiler Inspection Code, ANSI NB-23 .
