Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads: a) Warehouse Area; The following deficiencies existed with the steel frame racks: (1) The racks were not anchored to prevent movement. (2) The racks were not equipped with plaques showing the load capacities of the racks. Among other methods, a feasible and acceptable method to correct this hazard is to: (1) Anchor the steel racks as required to the floor or structural support members capable of preventing movement of the racks. (2) Install plaques showing the load capacities of the racks.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: a) Warehouse Area; The forklift was used to raise an employee to the top of the center rack while standing on an unsecured pallet on the forks without a guardrail. The employee was exposed to a fall hazard of approximately 14 feet. Among other methods, a feasible and acceptable method to correct this hazard is to use a platform with standard guardrails that is secured to the forks in a manner that would prevent it from coming off the forks while in use.e
