Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Loading Dock Area; Employees are exposed to a collapsing wall that deflects from ninety degrees at the foundation to between seventy and seventy five degrees at the roof line. They are also exposed to a collapsing roof at the warehouse.

General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) Facility; A forklift was used to raise an employee to a structural section of the ceiling to put an extension cord up. The employee was standing on the forks without a platform or guardrails. The employee was exposed to a fall hazard of at least ten feet. Among other methods, a feasible and acceptable method to correct this hazard is to use a platform with standard guardrails that is secured to the forks in a manner that would prevent it from coming off the forks while in use.
