Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling material: (a) Facility: The steel storage racks were not equipped with plaques showing the load capacities of the steel storage racks. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO INSTALL LOAD CAPACITY PLAQUES IN ACCORDANCE WITH THE AMERICAN NATIONAL STANDARDS INSTITUTE, ANSI MH 16.1, SPECIFICATION FOR THE DESIGN, TESTING AND UTILIZATION OF INDUSTRIAL STEEL STORAGE RACKS.E

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by a hoist load: (a) Reuss Engineers: The following deficiencies were noted with the 5 ton underhung hoist located in the shop: 1. The pendant control box was modified without the manufacturer's authorization. This condition puts the hoist operator in a pinch point between the load and a fixed object. 2. The hoist was not plainly marked on both sides as to it's rated load capacity. 3. The hoist was not fitted with rail sweeps. In addition, the company did not have the owner's/operator's manual for the hoist. 4. There were no signed monthly reports on the hoist hook. 5. There was no preventative maintenance program based upon the manufacturer's recommendations. 6. There was a defective limit switch on the hoist within the last 6 months. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO REPAIR AND DEVELOP A PREVENTATIVE MAINTENANCE SCHEDULE IN ACCORDANCE WITH THE MANUFACTURER'S RECOMMENDATIONS.
