Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to possible falling loads and/or falling hoist parts: (a) Facility, Building #3: The rated load was not permanently marked on the hoist. The operator of the hoist was not trained in the capabilities and load limits of the hoist, and the monthly hoist inspections were not thorough and written reports on the overhead hoists were incomplete. Among other methods to correct this hazard is to develop a training program in accordance with the hoist manufacturer's recommendations which addresses employee training and documented maintenance.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to possible falls: (a) Facility, Building #3: Employees are being elevated in a Simon manlift without wearing a safety harness. Among other methods to correct this hazard is to require the use of safety harnesses when employees are elevated in the Simon manlift.
