Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the potential hazard of fire while manufacturing flexible polyurethane foam buns and logs to be later used for carpet underlayment. The company did not employ the use of a high temperature alarm/detection system to alert employees of an unusual exothermic condition which could result in auto-ignition of curing polyurethane foam product. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to install and use needle thermocouples which when inserted into the interior of curing foam buns and logs allows the recording of the temperature and its change with time. The rate of heat evolved from the exothermic reaction(s) should NOT be greater than the rate of heat loss through the large bun/log of foam. A continuing increase in the interior temperature of foam buns(s)/log(s) during the first few hours after manufacture should sound an alarm.
