Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls to the ground: a.Forklift Platform - The platform's floor dimensions exceeded two times the fork separation distance of 24". b.Forklift Platform - The platform did not have a 4" toe plate. c.Forklift Platform - The platform did not have a plate with the following information on it: maximum work load including personnel and equipment; and weight of the empty platform. d.Forklift Platform - The platform did not have a means to ensure that the platform could be centered laterally on the fork truck. e.Forklift Platform - The platform was not secured to the vertical face of the forks, carraige or lifting mechanism. f.Forklift Platform - The platform did not have a means to securely attach the platform to the lifting platform to prevent the platform from pivoting. g.Forklift Platform - The platform was operated without rendering inoperative the controls other than the elevating controls. h.Forklift Platform - The platform's floor dimensions guardrails were less than 42 inches above the platform; the midrails were less than 21 inches above the platform. ABATEMENT NOTE: One feasible and acceptable method of abatement would be to comply with requirements of the American Society of Mechanical Engineers document ASME/ANSI B56.6-1992, Safety Standard for Rough Terrain Forklift Trucks.t
