Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to hazards of being hit/crushed from falling loads:
a.Dept. 99, Valve Assembly Bench - Employees were exposed to the hazards of
falling loads due to failure of the lifting device. The capacity of the
lifting device
was unknown, there were no markings, and was not load tested.
b.Dept. 80, Barrett Centrifuge - Employees were exposed to the hazards of
falling
loads due the failure of lifting devices. There were three lifting devices
in the
area with unknown capacities, there were no markings, and were not load
tested.
c.Dept. 73, Black Oxide Line - Employees were exposed to the hazards of
falling
loads due to the failure of lifting device. The capacity of the lifting
device
was
unknown, there were no markings, and was not load tested.
d.Dept. 91, Aqueous Degreaser - Employees were exposed to the hazards of
falling
loads due to the failure of the lifting device. The capacity of the
lifting device
was unknown, there were no markings, and was not load tested.
e.Dept. 77, Therm Lift - Employee exposed to the hazards of being
hit/crushed by
a falling load due to the failure of the lifting unit. The lifting unit
was not being
operated in accordance with the manufacturer's recommendations in that a
counter
weight was not in place.
f.Dept.78G, Barrett Portable Elevator - Employee exposed to the hazards of
being
hit/crushed by a falling load due to the failure of the lifting unit. The
lifting unit
had a damaged wire rope lifting line and the wire rope clips on the
lifting line
were not installed properly.
g.Foundry, Grinding Dept., Finish Wheelabrator - Employee exposed to the
hazards of being hit/crushed by a falling load due to the skip loader. The
skip
loader lifting rope was not adequately secured in that there were
inadequate
number of wire rope clips were used to connect the lifting rope.
h.Crane #25 - Employees were exposed to being hit/crushed by the crane
system
and/or load due to a crane failure. The crane was installed without
adequate
checks being made to determine if the building structure the crane system
was
attached to was adequate to support the crane bridge, crane trolley,
crane, and
loads being serviced.
i.Crane #51 - Employees were exposed to being hit/crushed by the crane
system
and/or load due to a crane failure. The crane was installed without
adequate
checks being made to determine if the building structure the crane system
was
attached to was adequate to support the crane system and loads being
serviced. j.Jib-type cranes throughout the facility - Employees were
exposed to being
hit/crushed by the crane system and/or load due to a crane failure. The
jib-type
cranes were not installed in accordance with the manufacturer's
recommendations.
ABATEMENT NOTE: Crane and hoist systems must be installed, used, and
maintained
in accordance with the manufacturer's recommendations or be certified by a
person
competent in crane and hoist systems and structural design to ensure they
are installed
properly to support anticipated uses.
