Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being crushed and/or hit by a falling load:
a.St. Paul's School, Concord, NH - The operator of a trolley boom delivery
truck
was exposed to being crushed and/or hit by a falling load due to the
inadequate
operation and maintenance of the trolley boom. Inadequate operation and
maintenance practices observed on the site include the following items:
1.The connection system of the pallet lifting device to the load line
was damaged.
2.The operator did not make adequate inspections of the lifting
system before operation and correct unsafe conditions before
operating the lifting system.
3.The operator was not properly trained in the operating and
maintenance of the lifting system and did not follow the
manufacturer's recommended operating procedures.
4.Load and capacity information for the lifting system was not
available on site.
ABATEMENT NOTE - Refer to the "Guide to Operating & Maintenance"  which is
the manual
for Side-O-Matic 60-18 CNE trolley boom manufactured by U. S. Truck Cranes
Inc for the
recommended operation and maintenance procedures.
