a.Throughout the Facility - Employees were exposed to possible crushing
hazards
while operating and working adjacent to cranes due to a possible crane
failures.
Possible crane failures due to an inadequate crane inspection and
maintenance
program in place to recognize and correct unsafe conditions which may occur
during normal crane operations.
One feasible method of abatement, among others, is to establish and
implement an inspection
and preventative maintenance program based on the manufacturer's
recommendations or based
onthe recommendations in the American National Standards Institute's
"Safety Codes for Cranes,
Derricks, and Hoists."
b.Foundry - Employees working with overhead cranes to lift and transport
pour
parts in the pouring area were exposed to possible crushing injuries while
lifting
and travelling with loads supported with custom-made lifting devices which
were
not approved lifting devices.
c.Pickle Room - Employees working with overhead cranes to lift and
transport parts
into and out of the treatment tanks were exposed to possible crushing
injuries
while lifting and travelling with loads supported with custom-made lifting
devices
which were not approved lifting devices.
One feasible method of abatement, among others, is to design, fabricate,
test, mark,and inspect
the custom made lifting devices in accordance with the American National
Standards Institute
"Below the Hook Lifting Devices."
