Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
A.Worksite.  Employees were exposed to various hazards, such as, but not
limited to, fire and explosion, while drilling for natural gas resulting
from
employees not receiving sufficient instructions in correct work methods to
reduce chance of injury to themselves or fellow employees.  Sufficient or
adequate instructions shall include training in areas such as, but not
limited to,
derrick operations, well control, fire and explosion prevention and
detection,
making and breaking drill pipe connections, drill pipe and casting, rotary
tables, tongs, well blowout, welding and cutting, blowout prevention,
hydrogen sulfide monitoring, PPE, respiratory, fire extinguisher, material
handling, first-aid/CPR training, bloodborne, oil spill containment,
hazardous
communication, lock-out/ tag-out,..etc....Two employees were severely burn,
and one employee received a leg injury.
Feasible and acceptable abatement methods to correct these hazards as
specified in API
RP54 92, page 9, paragraphs 5.1.2 through 5.1.5, such as, but not limited
to, familiarizing
employees with recommended practices as they relate to their function in
the job on hand,
instructions of the driller or crew chief specifically in his
responsibility for the safety of his
crew and equipment during normal operations and possible emergencies,
general safety
education through safety meetings, company publications, and other
educational media, the
offgoing tour supervisor shall inform the oncoming supervisor or any known
special hazards
or ongoing work that may affect the safety of the crew,....etc....

a.The hazard of fire and explosion resulting from a kerosene heater that
was
located and operating in the doghouse, within 75 feet of the well head.
One feasible and acceptable abatement method among others to correct the
hazards is
to prohibit kerosene heaters operating within 75 feet of the well head as
specified in
the NIOSH Document 83-127 entitled "Comprehensive Safety Recommendations
for
Land-Based Oil and Gas Well Drilling" Section 9.b.
b.Employees were exposed to the hazard of fire and explosion because of
employees smoking in areas such as, but not limited to, the immediate area
of
the well head in the Dog House, in their personal or company vehicle ...
etc...while drilling for gas.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD TO CORRECT THIS HAZARD IS for employees not to
smoke, or no opening flame within 75 feet of the well head, as specified
in the
NIOSH Document 83-127 entitled "Comprehensive Safety Recommendations for
Land-Based Oil and Gas Well Drilling" Section 9.b..
