Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries due to: (a)Beam Shed. The hoisting motion of the No. 1, Robbins & Myers, 5 ton radio controlled underhung crane, was not provided with an operable travel limit switch in the hoisting direction. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to comply with ANSI B30.11, Monorail Systems and Underhung Cranes, Section 11-3.2.4, by testing the limit switch at the beginning of each shift, under no load. If the switch is inoperable, the operator shall immediately notify the appropriate person for repair.

(b)The carrier (trolley) brakes, on the No. 7, Robbins & Myers 5 ton, underhung crane, were in need of adjustment. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS comply with ANSI B30.11, Monorail Systems and Underhung Cranes, Section 11-2.1.5, in that written, dated, and signed monthly inspection reports and records should be made on critical items such as brakes.

AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to comply with ANSI B30.11, Monorail Systems and Underhung Cranes, Section 11-1.3.5, in that cranes and monorail systems may be re-rated provided modifications in the trolleys, crane, track and supports are checked thoroughly by a qualified engineer or the equipment manufacturer.
