Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) On or about June 27,1997 at the east side of the Car Max building, an
employee raised on
the forks of a Gradall lift truck was exposed to a fall hazard of
approximately 16 feet.
Among other methods, one feasible and acceptable method to correct the
hazard is to follow the
recommendations of the American National Standards Institute (ANSI) B56.1-
1993-4.17.2 (a),
which requires the use of personal safety platforms on powered industrial
trucks to protect
employees from falls.
PLEASE NOTE:  CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS
REQUIRED FOR THIS VIOLATION.
