employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm
to employees in that:
Employees were exposed to crushing injuries resulting from forklift truck
tip over accidents:
(a) At this establishment located at 5801 Yahl Street, Naples, FL 34109,
the following was
found: 1. On or about January 15, 1998, the employer did not enforce the
use of seat belts
which were provided on the two John Deere 482C forklift trucks used to
remove, dump and
replace material sorting containers for the conveyor sorting line.  Among
other methods, one
feasible and acceptable abatement method would be to require fork truck
operators to use seat
belts which are provided on the fork trucks in accordance with ASME/ANSI
B56.1a-1989
Addenda to ASME/ANSI B56.1-1988 and ASME/ANSI B56.1-1993.
PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS
VIOLATION.
