Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to having any parts of their bodies caught in or between any parts of the mast (derrick) of a well drilling rig when riding on the head of the drill being raised aloft to grease fittings on the mast or appurtenances thereto: Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1) Never ride the head of the drill on a drilling rig: 2) Enforce the prohibition of not riding the head of the drilling rig by appropriate disciplinary action; and, 3) Shut down the drill rig engine, lower mast and deenergize the rig using proper lockout/tagout procedures before starting lubrication of fittings as referenced in the "Drill Safety Guide" as promulgated by the National Drilling Association. LOT 17, SPRING ST., LUDLOW, MA.: DRILLER'S HELPER WAS RIDING THE HEAD OF THE DRILLING RIG BEING RAISED UPWARDS TO GREASE FITTINGS AND/OR PULLEYS ON THE WINCH WHEN HIS RIGHT FOOT BECAME CAUGHT BETWEEN THE HEAD AND THE TROLLEY AS THE TWO WERE PASSING.
