recognized hazards
that were causing or likely to cause death or serious physical harm to

Tripping and falling in Warehouse areas where stock is stored due to
inadequate lighting on 5
levels in the warehouse sections.  On or about 2/20/98, Levels of
illumination measured
were 0.1 to 1.4 footcandles in rooms 2 and 3 on all levels. The levels of
illumination
measured in stairwells from the first floor level up to and including
floor level 5 located in
Room 1 and 3 was 0.0 footcandles up to and including 1.0 footcandles.
Among other methods, one feasible and acceptable means of correcting the
hazard would be
to provide lighting, as recommended by American National Standard Practice
for Industrial
Lighting, publication RP-7 (A11.1-1973) sponsored by the Illuminating
Engineering Society,
at a level of 5 footcandles in inactive storage rooms or warehouses, 10
footcandles in active
storage rooms or warehouses for rough bulky operations, 20 footcandles in
active storage
rooms or warehouses for medium operations, and 50 footcandles in active
storage rooms or
warehouses for fine operations. The level of illumination as recommended
by the American
National Standards Institute in the same publication listed above, is 20
footcandles for
stairways and corridors.
