Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards on July 7, 1998 during an acetone reclamation operation. The following conditions contributed to the existence of this hazard: a) BUILDING #2; IN AREAS WHERE FLAMMABLE LIQUIDS SUCH AS BUT NOT LIMITED TO ACETONE, STYRENE MONOMER, AND SEMI FINISHED RESINS ARE REGULARLY HANDLED, A MONITORING SYSTEM WAS NOT IN PLACE TO DETECT AND SOUND AN ALARM WHEN FLAMMABLE VAPOR AIR MIXTURES REACH 10 OF THE SUBSTANCES LOWER EXPLOSIVE LIMITS (LEL) SO THAT CORRECTIVE MEASURES COULD BE TAKEN TO PREVENT AN IGNITION.
