The employer did not furnish employment and a place of employment which
was free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees.
On or about February 16, 1998, Linder-Stahl Construction, Co., Inc. had a
crew of 5
employees working at 1200 Ball Parkway, Arlington, Texas conducting a
"Line Acceptance"
test; once completed, an employee was suited up in a body harness, to be
lowered down into
a manhole to deflate an Air-Loc #267-128 test plug that had been inflated
to 25 psi.  The
employer did not follow the manufacturers recommended guidelines or the
industry practice
guidelines from prohibiting employees to enter in to the "danger zone".
