Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards causing or likely to cause death or serious physical harm to employees. The employees working in the FRS Process System were exposed to explosion and exposure to hazardous waste chemicals from filter vessels which were not equipped with safety relief valves designed to safely relieve an over pressure condition in the siphon and other systems in the FRS Process: a) The S-151 filter vessel located along the north wall east of the T-160 receiver vessel was not equipped with a safety relief valve. b) The S-110 filter vessel located along the north wall east of the S-151 filter vessel was not equipped with a safety relief valve.
