The Employer did not furnish employment and a place of employment which
was free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees:
At the Oxygen-Fill Station, in Department 421 in the northeast corner of
the plant,
employees were exposed to an explosion hazard when filling medical oxygen
cylinders. The
system was not equipped with emergency shut-off valves and an alarm system
in order to
abort or shut down the system in case of an emergency.
On September 23, 1998, three employees suffered burns when oxygen entered
the vacuum
pump resulting in an explosion.
FEASIBLE MEANS OF ABATEMENT:
One feasible means of abatement is to install emergency shut-off valves on
the oxygen line
and to install a visible or an audible alarm in case of an emergency and
the line had to be
shut off.

employees:
On September 23, 1998, the oxygen fill station located in the northeast
corner of the
plant in Department 421 where three employees suffered burns when oxygen
entered the
vacuum pump resulting in an explosion.  The Oxygen-fill station was not
isolated from other
areas of the plant.
FEASIBLE MEANS OF ABATEMENT:
One feasible means of abatement is to install the oxygen fill station in a
room by itself with
the appropriate hazard warnings posted and having at least two means of
egress.

vacuum pump was not equipped with a relief valve or any other type of
safety devices to
prevent oxygen from entering the pump and creating an explosive mixture
with the pump oil.
The employer did not follow the guidelines as outlined in NFPA 99C 1996 or
as
per industry
practice.  The employer did not follow their own requirements as was
required in the
employers "USP Oxygen Fill Process Validation Protocol" that was signed by
management
on September 4, 1998.
On September 23, 1998, three employees suffered burns when oxygen entered
the vacuum
pump resulting in an explosion.
FEASIBLE MEANS OF ABATEMENT:
One feasible means of abatement is to install a relief valve on the oxygen
line to prevent the
oxygen from entering the vacuum pump and creating an explosive mixture.
