Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a.The employer did not design and assure the structural integrity on the
hoist (underhung)
supporting structure auxiliary beams and welds.  These are intended to
take a shock load
force from a suspended load (1100 to 1400 pounds).  Employees are expected
to work
under a suspended load exposing employees to struck-by hazards.
Among other methods, a feasible and acceptable abatement method to correct
this hazard
is to comply with the safe operating procedures recommended by ASME/ANSI
B30.16
such as, but not limited to:
-Acquire the services of a registered professional engineer to evaluate
and
certify
that the existing configuration meets the design criteria for structural
integrity.

a.The employer did not inspect the critical components of a hoist such as
hooks, controls,
limit stops, chains, suspension members, brakes, load and idler sprockets
and supporting
structure or trolley which collectively lift loads (1100 to 1400 pounds).
Employees are
expected to work under a suspended load thereby exposing employees to
struck-by
hazards.
Among other methods, a feasible and acceptable abatement method to correct
this hazard
is to comply with the safe operating procedures and inspection
requirements recommended
by the manufacturer and the ASME/ANSI B30.16 such as, but not limited to:
The company has the hoist owners manual inspection guides on hand at the
site
but failed to assure that a qualified person performs the inspections,
maintains the
upkeep of hoists; and trains the operators to perform daily service checks
The company can use the ASME/ANSI  the American Society of Mechanical
Engineers/ American National Standards Institute ASME B30.16-1993 (Overhead
Hoist (Underhung)), but will have to assure that a qualified person
perform the
inspections, maintains the upkeep of hoists; and trains the operators to
perform
daily service checks.
Or contact a local crane/hoist company to perform inspections and maintain
proper
up keep.

who work under
suspended load (1100 to 1400 pounds) thereby exposing employees to
struck-by hazards.
Among other methods, a feasible and acceptable abatement method to correct
this hazard
is to comply with the safe operating procedures recommended by the
manufacturer and
the ASME/ANSI B30.16 such as, but not limited to:
-Install the  n shaped type setup on all of the rotomold platforms where
the
weight does not rest on the platform but is transferred directly to the
floor.  This
type of frame will have to extend over the entire operation to protect all
exposed
employees from the load suspended above them.
