Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment which
were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that:  On or about February 5, 1997, employees were exposed
to falls from
elevations while operating a truck mounted aerial lift bucket with a
defective door hinge.
NYNEX vehicle #22-28022 equipped with a TEL-29-EGA bucket(serial # F19216)
had missing
bolts/rivets in the door hinge.  Among other methods, one feasible and
acceptable method to
correct this hazard is to establish and follow a preventative maintenance
program which contains
at least the following:
1. Ensure that all manufacturers recommended maintenance inspections are
adhered to.
2. Ensure that all aerial lift vehicles are inspected by a qualified
inspector from the manufacturer
at least once a year and the inspection documented prior to placing the
vehicle
back in service.
3.  Establish a procedure where the manufacturer's maintenance inspector
is accompanied by a
management representative of the corporation during the entire inspection
and approves the
inspection report prior to placing the aerial lift back in service.
4.  Ensure that a copy of the inspection report is given to the first
level supervisor assigned to
the vehicle and reviewed with the driver assigned to that vehicle.
5.  Ensure the corporation adheres to a strictly enforced inspection and
repair policy. for repair
of items that were observed during the inspection that needed
replacement/repair.
