Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of falling from an elevated work station while
connecting a steel
beam to a steel column:
(a) Location - 40 Bailey Blvd., Haverhill, MA:
On or about December 15, 1997, an employee fell 42 feet while in the
process of
connecting a steel beam to a steel column when a single bolt was placed in
the bottom
hole of the flange which acted as a pivot point or hinge.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be to place two bolts, one in the top hole and one in the bottom
hole of the column clip
and beam flange.  In the alternative, there should be two employees
performing this operation
one on each end of the beam.
