Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a) Location - Building 26, Floors 2 through 6:
On or about 7/22/98, holes in floors through which conveyors pass had the
potential for
allowing smoke to channel rapidly up from one floor to the next in the
fashion of a
chimney under fire conditions.
Among other methods, one feasible means of abatement would be to provide
covers to
fit over the opening in the floor and the conveyor through which smoke
could not pass
and to only open the cover on any one floor while the conveyor is in
actual use.
