Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of being struck or crushed. (a) Location 96 Swampscott Rd., Salem, MA: On or before 09/23/98, employees were exposed to the hazard of being struck or crushed by the structural collapse of the rack storage system. Among other methods, feasible and acceptable methods to correct this hazard are the following: 1. Follow the rack manufacture's instructions regarding proper rack storage and usage. 2. Only modify rack system per manufacture's approval. 3. Inspect and repair damaged racks frequently to prevent a collapse.
