Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees
in that employees
were exposed to fire hazards:
(a) Location - Ink Room:
On or about 10-28-98, a room with connecting walls and doorways to other
portions of
the building was used primarily for the storage of flammable liquids, but
which was also
used for dispensing and mixing of Class I flammable liquids.  The room was
over 1,000
square feet, did not meet the safety requirements of a cut-off room, and
did not cut off
the dispensing and mixing area from the storage area with construction of
adequate fire
resistive ratings and contained larger quantities in the room than is
considered safe.
Among other means, feasible means of abatement would be to reduce the
amount of
Class I, II, and IIIA flammable/combustible liquids in the room to a
combined quantity
considered acceptable for safety purposes, move dispensing and mixing
operations out
of the room into one designed for that purpose,  and to provide the
following
design and
construction characteristics to the room:
1.  A fire-rated wall between the room and rest of the building;
2.  Spill containment to prevent liquid spill from flowing into the
connected portion of
the building.
3.  Liquid-tight wall/floor connection at the wall shared by another
portion of the
building.
4.  Approved self-closing fire door to connecting portion of the building.
5.  Make-up air ventilation duct into the room be provided with a fire
door or damper
6. Exhausts ducts for the ventilation no more then 12 inches from the
floor.
7.  An airflow switch in the ventilation connected to an audible  alarm
for ventilation
failure.
8. Class I, Division II, wiring and electrical equipment in the room.
(b) Location - Urethane Compound Room/Hot Room:
On or about 10-28-98, a room with a connecting wall and doorways to
another portion
of the building was used primarily for the storage of flammable liquids,
but which was
also used for dispensing and mixing of Class I flammable liquids.  The
room was over
1,000 sq ft.,  did not meet the safety requirements for a cut-off room,
and did not cut
off the dispensing and mixing area from the storage area with construction
of adequate
fire resistive ratings and contained larger quantities in the room than is
considered safe
Also, one separate part of the room was used for storage of flammable
liquids
undergoing a slow polymerization(hot room).Among other means, feasible
means of abatement would be to reduce the amount of
Class I, II, and IIIA flammable/combustible liquids in the room to a
combined quantity
considered acceptable for safety purposes, move dispensing and mixing
operations out
of the room into one designed for that purpose, move the material
undergoing
polymerization, and to provide the following design and construction
characteristics to
the room:
1.  Repair the hole in fire-rated wall to revalidate fire rating.
2.  Spill containment at loading dock and to outside area.
3.  Approved self-closing fire doors to connecting portions of the
building.
4.  Make-up air ventilation duct into the room be provided with a fire
door or damper.
5.  Exhaust duct for the ventilation no more than 12 inches from the floor.
6.  An airflow switch in the ventilation connected to an audible alarm for
ventilation
failure.
7.  Class I, Division II, wiring and electrical equipment in the room.
(c)  Location - Basement
On or about 10-29-98, Class I flammable liquids in containers were stored
in the
basement along with Class II and III flammable liquids without adequate
sprinklers.
Among other means, feasible means of abatement would be to:
1.  Remove all class I flammable liquids from the basement;
2.  Provide adequate sprinkler protection for class I and II liquids.

(a) Location - C-1 Line:
On or about 11-5-98, employees were working from a platform which was
elevated by
the forks of a powered industrial lift truck and the platform was not
secured to the forks
and midrails were not provided on the platform.
Among other means, feasible means of abatement would be to provide a
platform which
is securely affixed to the forks of the truck and provided with a full
guardrail system
which includes both a top and midrail.
