Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of delayed rescue and possible injury during
entry into and exit from
water treatment tanks.
(a) Location - Cambridge Water Treatment Plant:
On or about 06/27/00, the employer had not developed and implemented a
Confined
Space entry program that included rescue of employees injured or overcome
by oxygen
deficiency or other toxic air contaminants while removing concrete form
lumber,
performing concrete finishing work such as chipping and grinding.
Among other methods, feasible means of abatement would be to institute a
confined
space entry program meeting the requirements of OSHA 1910.146 or ANSI
Z117.1-
1989.  The program should include:
1)  Use of an effective retrieval method such as body harness, tether
line, and winch
system.
2)  Continuous air monitoring where there is a potential for oxygen
deficiency or entry
of toxic or hazardous air contaminants.
3)  Effective training of the authorized entrants, attendants, and all
affected employees.
