recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to:
On December 3, 1999, employees making organic castings or paints and using
flammable and combustible liquids in the 3rd floor, plant 1 manufacturing
room were
exposed to fires and explosions in that electrical switch gear (ignition
sources) located
in the north west enclosure or room was not safeguarded by positive
pressure
ventilation designed to prevent entry of flammable vapors, that is, the
ventilation
system was inoperable.  Also, when the positive pressure fails or is not
maintained,
the protective enclosure was not provided with an alarm or indicator which
shall
warm the user to take action or shall automatically de-energize the power
from
ignition, capable equipment.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE
ABATEMENT METHOD              TO CORRECT THIS HAZARD IS...."
These hazards is to design and construct the enclosure, electrical
equipment and
switch gear using positive pressure ventilation that is maintained by
devices such as
alarms, indicators, cut-offs or interlock switch which warn the user to
take action or
shall automatically de-energize power from ignition-capable equipment as
stipulated
by NFPA 35, standard for the manufacture of organic coatings 1995, chap
3-6.4 and
NFPA 496 standard for purged and pressurized enclosures for electrical
equipment,
1998, chapter 2-3.2.
